To qualify, a patient must:
Be under the care of a physician
Have a medical condition requiring skilled nursing or therapy services
Be homebound, meaning leaving the home requires considerable effort or assistance

“Homebound” doesn’t mean the patient can never leave home—it means they are unable to leave home without a considerable effort, supportive device, or help from another person. Brief and infrequent outings (such as medical appointments, religious services, or short walks) are still allowed (United States Department of Health and Human Services, 2004).
Your doctor or allowed practitioner documents that they’ve had a face-to-face encounter with you (like an appointment with your primary care doctor) within required timeframes and that the encounter was related to the reason you need home health care (United States Department of Health and Human Services, 2004).
Home health care usually begins with a referral from a physician, hospital, or healthcare provider. Once we receive the referral, we will contact you or your family to schedule an in-home assessment and begin services quickly. Yes. Medicare, Medi-Cal, and most private insurance plans cover eligible home health care services, provided certain conditions are met. Our intake and billing team can assist with benefit verification and authorization.
